In 2007, the Judicial Council made significant changes to the forms for name and gender changes. All of those new forms, and instructions for completing them, are included in the 12th edition.

As of January 1, 2008, the official California notarization statement changes to the following:

I certify under PENALTY OF PERJURY under the laws of the State of California that the foregoing is true and correct. Witness my hand and official seal.
