Use this form if you are the executor or personal representative (the person in charge of wrapping up the deceased person’s affairs) of the estate of a person who owned and insured one or more automobiles, trucks, motorcycles, motor homes, or trailers. It informs the insurance company of the death of the insured person and asks them to add you, the executor, to the insurance policy.
