No "Rules for Effective Discipline" Checklist

In Appendix A of the second edition of the Performance Appraisal Handbook, we list the contents of the CD-ROM accompanying the book. That list erroneously includes an item that isn't on the CD or in the book, "Checklist: Rules for Effective Discipline." If you try to open this file on the CD, you will see only a blank PDF form.
We removed this item from the second edition because we now have an entire book dedicated to employee discipline, The Progressive Discipline Handbook.
We apologize for the confusion.
