Within one working day after receiving notice or knowledge of your injury, your supervisor will give you a claim form for you to complete and return to him/her. The supervisor will give you a dated and signed copy of the claim form and will forward a copy to the workers' compensation insurance carrier.

Contact your doctor to find out if a report was sent to the workers' compensation insurance carrier or your self-insured employer.
